Social Work Case Manager (LMSW/LCSW) - Emergency Department, Maternal Child Health

As a member of a multidisciplinary health care team, the Social Work Case Manager provides psychosocial interventions to patients, as well as their families and decision makers, receiving care within the health system and across the care continuum. The Social Work Case Manager conducts psychosocial assessment of patients/families as clinically indicated to evaluate social, emotional, and physical needs by applying social work theories and techniques.

The Social Work Case Manager Collaborates with interdisciplinary health care teams to devise, develop, and implement patient/family psychosocial treatment plans, coordinating referrals to specialty and social services to support continuity of care, recording thorough progress notes and interactions with patients/families/providers/agencies as indicated.

This position will focus on patients of the emergency department (ED) as well as those within the Maternal Child Health (MCH) Unit - coordinating with outpatient practices, providers, and resources to identify and address barriers to self-management.

Job Responsibilities include:

  • Supports patients/families to cope with environmental and psychological issues of illness.
  • Manages independently assigned caseload of varying clinical acuity.
  • Attends team conferences and participates in interdisciplinary rounds and unit-based staff meetings as able.
  • Provides supportive counseling, grief counseling, crisis intervention, and other therapeutic services to assist patients and families to face challenges such as coping with illness, processing medical information, and changes in the level of functioning and care needs.
  • Participates in quality assurance and data collection as assigned.
  • Assess the patient's health insurance plan and facilitate insurance navigations as indicated.
  • Facilitates completion of advanced care directives as indicated.
  • Appropriately documents client interactions and intended plan of care.
  • Routinely communicates with patient family/caregivers/decision makers the updated plan of care.
  • Monitors and appropriately escalates issues affecting patient safety and/or LOS. Actively intervenes to facilitate patient progression and safe care transitions.
  • Works in collaboration with ancillary personnel (i.e. physical therapy, pharmacy, diabetes education) to support identified patient care needs.
  • Follows regulatory guidelines and completes associated authorizations for transportation and related transitions of care to alternative agencies/levels of care.
  • Assists in the delivery of timely notifications to allow patient and their care teams to make informed decisions regarding the plan of care.
  • Demonstrates emotional stability and organizational skills necessary to meet the demands of the busy healthcare environment.
  • Flexible in taking additional assignments as needed to cover for planned and unplanned events across the system and appropriately shares knowledge with coworkers.
  • Seeks learning opportunities to optimize job performance.
  • Participates in system taskforces and other collaborative activities with community partners to support facility growth and transitions of care.
  • Maintains high visibility and is readily approachable and accessible to care teams.
  • Remains calm in stressful scenarios and able to resolve conflict independently, appropriately escalating concerns as indicated.

Required Skills and Experience :

Education/Licensure - Masters of Social Work program and a ctive NYS social work licensure.

Experience - Minimum 2 to 3 years' experience in an acute care hospital setting or similar health care case experience.

Knowledge of community-based organizations and strong communication, organizational, critical thinking and problem solving skills required.

Computer literacy and knowledge of EMR systems.

Demonstrates resilience in coping with challenging situations.

Physicality - Ability to stand, sit or ambulate for long periods. Ability to mobilize freely around units and perform required data entry into EMR and alternate data management systems. Ability to transport self to customer base

Pay Range Disclosure:

$80,080.00 to $107,120.00 annual salary
